Serving the cities of Corning, Geneva, Elmira and Rochester, and their surrounding counties, the Alzheimer’s Association Rochester Finger Lakes Region Chapter is here to help. We provide education and support to all those facing Alzheimer’s and other dementias throughout our community, including those living with the disease, caregivers, health care professionals and families. We are also committed to advocating for the needs and rights of those facing Alzheimer’s disease and all other dementia, and advancing critical research toward methods of treatment, prevention and, ultimately, a cure.
When caring for someone with Alzheimer's disease it is important to plan. One of the benefits of planning in advance is that the person can participate in the decision-making process and have their wishes known. It often becomes difficult and divisive to make key decisions well after a person has been diagnosed with Alzheimer's disease. Planning in advance will help ease the transition in early retirement, financial planning, healthcare, and safety and security issues.
Lifespan has an array of services that can help caregivers navigate care options and get support.
A loan closet is a program that allows people to borrow durable medical equipment and home medical equipment at no cost or at low cost. The loan closet may be offered through an organization, an individual, or some other entity, often a non-profit organization. Because medical equipment is expensive and often needed for only a short time, loan closets help people receive equipment that they may not otherwise be able to afford.
RCIL provides information, referrals, and advocacy to people with disabilities who are navigating the Human (Social) Services and Social Security systems. They understand trying to comprehend what is entailed with NYS disability and social security disability can be difficult and confusing. They assist to ensure people are knowledgeable about the systems and recognize the benefits they have the right to receive under the American Disabilities Act.
The Department of Human Services has as its goal a common-sense human service delivery system that is comprehensive, responsive, coordinated and based on measurable results. The department provides various forms of assistance to citizens of Monroe County including income support, medical assistance and comprehensive services designed to promote a higher quality of life for children, families and disadvantaged or elderly adults.
The Home Energy Assistance Program (HEAP) is a federal grant program that assists income eligible households in paying for energy costs, repairs, and weatherization. Income eligible customers may receive one regular HEAP benefit per season. In addition to regular HEAP, households may also be eligible for emergency HEAP benefits.

The options for meal services have expanded over the last several years. While traditional meal delivery services still exist, nationwide services for quick, easy-to reheat meals and meal kits which provide all the ingredients have expanded. Additionally, there are delivery options from local
Basic home delivered meals for people that have difficulty preparing their own meals. Deliver a hot, nutritious meal and provide a daily check-in to the home bound, elderly, or ill through a dedicated volunteer force. Eligibility for Meals on Wheels:18 years of age or older *Incapacitated due to accident, illness, or frailty. Living alone or with someone who is unable to prepare meals and/or is absent during the day. Receive meals a minimum of three days per week.

SNAP recipients can swipe their benefits card at a Market Market Token Program participants also receive a 40% bonus through N State Fresh Connect Program (up to a $20 bonus per SNAP recipient, per market day.

Supplemental Nutrition Assistance Program - SNAP provides food benefits to low-income families to supplement their grocery budget so they can afford the nutritious food essential to health and well-being.
HIICAP works directly with Medicare beneficiaries to educate them about the Medicare Program, Medigap policies, Medicaid, Medicare Advantage Plans, Long-term Care Insurance, Low-Income Subsidy Programs, employer-sponsored insurance, and other health insurance programs that are available in New York State. This program offers free and unbiased information on all Medicare options available within Monroe County. HIICAP also provides information on Medicare cost reduction programs MSP (Medicare Savings Program), EPIC (Elderly Pharmaceutical insurance Coverage) , Extra Help for Part D (LIS- Low income Subsidy), to those who qualify.
New York State of Health is a Health Plan Marketplace. Individuals, families, and small businesses can use it to buy health insurance. It lets you shop and compare many health plans. It is the only place to get help lowering the cost of health coverage. Those eligible for Medicare should contact Local HIICAP or Medicare directly with questions and program options.

Housing Advocacy Services works with Monroe County residents with hardships such as housing and homelessness, specifically, people with disabilities and dissociated populations. Provides case management, representative payee services and networking with other community providers in the greater Rochester area. Services include: Housing: Connections for Rapid Rehousing, Permanent Supportive Housing, Affordable Apartment Leads; Advocacy: Housing, Health Insurance Assistance, Free Community Medical Services, Apartment; Referrals: Treatment referrals along with Community Resources Intervention; Aid Services: Representative Payee, Case Management, Housing Referral Accessing Community; Supports Such As: Social Services, Health Care, Mental Health Services, Substance Abuse Treatment, Furniture assistance upon request
The Monroe Housing Collaborative was formed with local nonprofit agencies to improve the lives of people with disabilities by addressing the growing need for integrated, affordable housing throughout Monroe County. Al Sigl Community of Agencies, CP Rochester, and EPI have come together to gather the residential needs and preferences of those with disabilities, while working to expand living options in the Greater Rochester area.

Primarily assists in areas of domestic relations, housing/ landlord in private situations, immigration and youth advocacy.

They also complete minor home modifications including installation of grab bars, smoke detectors, handrails for stairs, door grips, handicapped toilet seats, tub transfer benches, bathtub seats. This service is a donation and income based in Monroe County. This is a program through lifespan.

Monroe County has many transportation options available for older adults and individuals with disabilities for medical appointments, errands, and social events. These options include services offered by volunteers, public services, private vendors, and medical transportation providers. Through Lifespan’s TRAC (Transportation Access) Program, mobility specialists can help Monroe County residents, or their caregivers explore options available to them. (TRAC is not a transportation provider, but rather a service to explore transportation options.) Transportation needs to start in the county the person resides in.

Monroe County Veteran Service Officers offer free assistance in navigating VA bureaucracy, including claim support, filing, and monitoring. They can also help with appeals. Use their services to avoid paperwork pitfalls and delays.
